This registration session includes:

• Contact information - including emergency contacts
• Medical information and concussion waiver
• USA lacrosse membership verification
• Program selection
• Uniform ordering
• Player pack sizing
• Concussion information
• Code of Conduct and Liability Waiver
• Payment - includes immediate payment in full, payment plans and personal check option

There is no volunteer buyout option. Instead, all families will need to write out a $400 volunteer check. If you fulfill the required 10 volunteer points, we'll return or destroy the check. If you don't, we will cash the check.

Please read through each page carefully to ensure that all information is provided accurately.
 

Your fee includes a $100 NONREFUNDABLE deposit. If you are paying by check (offline), you can make a check payable to "Mukwonago Lacrosse Club" and mail it to the address on the home page by December 15, 2023, or upon registration if after registering after Dec. 15.

You must register first with USA Lacrosse and then return to this Registration Session to signup.  Your USA Lacrosse registration must be good through June 30, 2024, to cover the entire 2024 season.

 
IF YOUR ATHLETE DOES NOT HAVE A CURRENT USA LACROSSE MEMBERSHIP: 

• CLICK HERE to go to USA Lacrosse Registration and register your player(s).
• Be sure to note the Member ID for each player.
• Remember to register EACH of your players with USA Lacrosse. Each player must have their own unique USA Lacrosse member number.

TO RENEW A USA LACROSSE MEMBERSHIP: 

• CLICK HERE to log in to your USA Lacrosse account.
• Check your membership expiration date. It needs to be after June 30, 2024.
• If the expiration date is before June 30, 2024, you will need to renew your membership. We do NOT require age verification for our program.

Once complete, return to this registration and proceed. Later in this registration session you will need to enter your USA Lacrosse member ID, and the form will verify membership through June 30, 2024.
